Please arrive on time for appointments. If you are over 15 minutes late for your appointment it may be rescheduled to the next available appointment.
If you must reschedule an appointment, we require a 24-hour notice.
You will be billed a $25.00 no show fee for cancellations without 24-hour notice or for missed appointments without notice.
You are expected to pay your medical bill at the time of service. Appointments will be rescheduled if unable to pay at the time of service.
If you have an outstanding balance, you will be notified at your office visit, and you will be expected to make arrangements with the billing office for a payment plan.
Please call during regular business hours for routine non-emergency requests and questions.
If you have a medical emergency, please call 911 or go to the nearest emergency room.

If you need to contact the on-call physician during non-business hours, please call the main office number at 770-658-2081 and leave a message. The on-call physician will return your call.
You are required to remain under the care of a primary care physician (PCP) as part of your outpatient pulmonary treatment program.
Non-pulmonary medical problems must be addressed by your PCP.
Disability paperwork takes a minimum of 5-7 business days to complete. An office visit is required for the physician to complete any paperwork if not seen within the last 90 days. We reserve the right to charge a fee for the completion of these forms.
Returned checks must be paid in full within 10 days of notification, plus a $30 fee.
